Found my issue! It was TextureCachePurge. And I understand what exactly happens, every 3 days it runs a texture clean and due to Fashion Sense, a mod its incompatible with, clears the player heads on the save screen in Multiplayer, causing the saving screen to 'Crash'. My issue is finally resolved!

Wife and I have put hundreds of hours into multiple saves now that disconnect at end of the day, and now the whole save is gone. Xbox series x. Anyone else having this issue?
