This secret note means there is a hidden place somewhere that is currently locked/hidden and that will become accessible later once you've made some progress in the game.
This works similar to the Summit which is locked until you reach 100% perfection in Qi's Perfection tracker. Though this time, this is Grandpa who has a secret area. Conditions to unlock it are not quite clear (at least to me), but the place holds another tracker (and thus related challenge) and new rewards.
I won't say more as I think it is best that you experience the same "What it is, I've seen this before… Let's see…" that I had a few hours ago.
