Windows PS4 Controller randomly stops working on PC

My PS4 controller will randomly disconnect when playing the game (whether I'm playing via bluetooth or wired). Usually the game will lag for a second and then it will disconnect. Once this happens the only way to fix it is to reset the game. Doesn't matter if it's bluetooth or plugged in, I've had it disconnect even when the light on the back of the controller is blue. The controller will also disconnect during the game if I plug or unplug any devices from my PC. I've also found that usually after it happens once, even when I reset the game, the controller either still won't work (and I'll have to reset again) or it will disconnect again pretty quickly. This usually happens a few times as well before it works again.
