It's always after I get the journal update for 25 floors. A few floors or so after that. It's always when I'm going down a ladder, staircase or a hole.
I'm playing the latest update with the latest patch of 1.6. Playing on steam with PC and mouse. No mods. Nothing else strange or unusual that I can think of.
It's really frustrating because I'm trying to get to level 100.
