Didn't see this posted anywhere, so wanted to drop it here: If you start the Shrine of Challenge quest in the Walnut Room, but go to Skull Cavern at any time during the challenge, it seems to complete the quest and doesn't show a shrine anywhere on level 120. I have been trying to get the same challenge back for a few seasons now, but it won't show up.
