General Suggestions

1. Make Zuzu city that players can go there using the bus station.
2. Let children grow more and not stay kids.
3. When the kids grow up on a certain age, they can attend school in the Zuzu city, and the player should afford the education fees.
4. Add some disease that the player would het infected and go to the clinic (like flu, vomiting, diarrhea, etc..), so the clinic would have more importance in the game.
5. Add more talking sentences to the NPCs.

Most of them are pretty common suggestions. At least we DID get more dialogue in the 1.6 update. Not a major overhaul, but a lot of little things.

Why the Zuzu city and not with Penny? Expand the museum to have a classroom or build a whole separate school building as recently been suggested. Farmer's kid's attending with Leo, Jas and Vincent would make the above even more reasonable.

The clinic should be made more important, I never buy anything there, unlike all the other shops. It's fogettable that Harvey even sells something. But I'm not sure if there's a way to implement an illness without it being too annoying. What kind of debuffs would it bring? Waking up late? (Doesn't exist in the game, even if you pass out you get 4 hours of sleep). Reduced energy and speed maybe?

some of the old harvest moon games had a mechanic that if you were outside too long in the rain, you’d be sick and stay in bed the next day. So either you couldn’t do anything on the rainy day (I never figured out how long it was, but just rushing over to the animal buildings to take care of them was ok. Harvesting some fields was too long.) or you couldn’t do anything the next day (including taking care of the animals, which made them grumpy and not produce). It was a very annoying mechanic.

when I first started Stardew I expected the same and was really careful about going out in the rain.

Never played any harvest moon games so I didn't even think about anything like that when going out in the rain. Shows how other games influence our expectations I guess. When I first started playing, I was confused as to why torches aren't crafted with coal like in MC.

Im new to this site so sorry if this reply is formatted oddly. I like the idea of skckness being implemented but only if its in the players control. For example having a chance to get sick if you dont make it to bed multiple days in a row. You could see harvey with a small cut scene playing stealing half an hour of your time and you walk out with no debufs. Say sickness makes you move slightly slower and have slightly worse luck so its not even that bad if you ignore it. Just an idea ofc lol

Nope it's perfectly fine!
Then it'd need a debuff icon actually telling you that you're sick and better go see Harvey. Otherwise it might be not that noticeable/if you do notice it you might not know why you're under that effect.
Also, let Harvey sell something you can consume to cure yourself without involving him. Just to make the shop slightly more useful. And the cutscene should probably also take away a bit of money for the treatment (?)
Not sure if the sickness shout get worse if you ignore it for long enough/keep passing out. Makes sense logically, just gotta not step into 'annoying' territory.

IF staying outside in rain to long makes the player sick, there could also be a game mechanic for rain clothes (beside use of medicine) to avoid it.
Meanwhile we have the Mannequins, so changing clothes would be easy. And it would give clothes much more use...
