It should not just disappear.

If you lose it, it will either reappear in the lost & found box in the mayor's mansion (unless you're on mobile, which didn't receive the last update yet), or it could reappear in a random chest. Also check fridges and other storage places.

It helps me to not lose it if I pick a special spot in my inventory where it always goes. That way I'll know if it gets lost rather quickly, and I'll have a better chance of finding it faster. Maybe that will help you in the future! :)

Somewhat similar, I find the way I most often misplace my tools is by putting them in a random box. Checking every box you have down on every part of the map is a pain (especially if you're as maniacally chest happy as I am, and as much of a hoarder), but I don't want to always carry around every tool because of inventory constraints. So I have a chest near the stairs of my farm specifically for putting in tools I don't need, like the watering can, or axe, or hoe, or fishing rod, or pickaxe, or whatever. Then if I find I don't have one of those, I know exactly where it is.

By being diligent about only ever having tools in my inventory or in that specific chest, I don't tend to run into problems anymore. Or at least, not often, and when I do, it's far more likely than not that Clint is hanging onto it for me and I just forgot to pick up the upgraded version at some point XD

See, while I can be kind of chest happy myself, I try to limit how spread out my chests are. That makes searching them easier.
