I bought the game a while back, but after opening it and punching everything out, I discovered that it was missing chits sheet seven. Googling what to do about it, I found this form to request the missing pieces:

I filled it out immediately, but have never received any sort of communication about it, and have not received the replacement sheet.
Recently, I decided to try filling out the form again in case there was a glitch, but still haven't received any communication about it and haven't received a replacement sheet.
Are they still being provided? It's really sad to have invested in this game and not be able to play it.
