A question for quests.

In some quests or errants requested by npcs it may happens that you have the object requested
but when you hand it to them is not accepted as part of the quest-errant but as a gift because presumably
they want you to start gathering the resource from the time you accepted the gift and onward.

ps: If you swim in the spa pool could you use an obelisk inside the pool and get teleported to the ginger island wearing your
cool swimsuits ? (ok little far-fetched by it was an idea of my kid that wants to have suitable clothes in the ginger island beach!!)

In my experience, quests that are not fishing ("overpopulation" posted by Demetrius, "keeping the art of fishing alive" by Willy), mining (Clint), or resource gathering (Robin), will accept an item you already have; the simple "I'm looking for [item]" can be completed without finding a fresh item.

I tested this, because it was an interesting thought and I'd just crafted a couple of island warps.

Sadly, the answer is no. The game will not let you use the totem while in swimwear (I tried while in the pool and while out but still in the spa area), only after you go back through the magic changing area and are back to your regular outfit.

You used to be able to escape the spa in swimwear but it was incredibly cursed.

You would be stuck in the swimming animation while going around town instead of walking, you'd constantly regenerate health and energy so it was amazing for the mines or random tasks around the valley. Didn't work in sc because the bus removed the effect and the desert warps didn't exist back then but was still incredibly fun, even just for laughs.

There are good stand-ins for beach clothing as for the Ladies you can Tailor 2 choices of tops in the Bikini Top (Tailor the Rainbow Shell) and Island Bikini (Golden Coconut). The only and bestest Guys top is the Fake Muscles Shirt (Muscle Remedy), which you Dye to match your skin color. Instant hotbod! Use the Shorts (Blueberry, Grape, Pepper, Melon) for the Ladies bottom, which works well for the Guys too or you can use the Baggy Shorts (Orange, Peach) for a more boardshorts look.

All of these are Dyeable, Muscles Shirt being the most important, so you can choose what colors you want. Bonus points if you Dye the Muscles Shirt a little lighter than your skin tone for that authentic Farmer Tan look.

Oh it absolutely was, nothing beats wandering around down as a dismembered head in a floating pond
I think I did that in Guild Wars once. (There was a transform potion that gave you the appearance of a random monster for a while, one of which was a big floating head. No pond though, but I think there were tentacles. Haven't played in forever.)
