I just reinstalled the game and started up a new file. I've had two instances of newly discovered items not showing up in my inventory.
The first was when I took 7 geodes to Clint. The first five was normal junk. These all went into my inventory just fine (I checked to make sure afterwards--they were still there). The sixth was a dwarf helmet, my first artifact. The seventh was limestone I think (also a new discovery). When I exited the geode breaking menu, I got a message saying how I found an artifact and Gunther would surely like to see it. When I got to the library to submit it, I realized the limestone wasn't in my inventory, though the dwarf helmet was. I had plenty of space in my inventory that it should've been able to go into when breaking geodes.
[Me at Clint's (I went back to see if somehow the limestone was on the floor), and me in the library after donating the dwarf helmet. No limestone...]
The second incident was when I was fishing. I got seaweed for the first time, but my inventory was full. It had the normal screen where I'm given the option to make space, so I did (I think I put something in the trash can). ....Then the seaweed was gone. I can't remember how this screen normally works, if I have to exit the menu, if throwing something out makes the new item automatically take its spot, if I have to manually drag it from that little extra slot above my inventory, nor do I remember what I did after throwing out the junk item cuz I wasn't paying that much attention--just going through motions to get seaweed in my inventory and leave the beach. Well. I was left with one empty slot and no seaweed. It wasn't on the ground either. So...I do have a bit of uncertainty here that maybe I did something wrong (like if I have to move the seaweed from that slot into my inventory before exiting or else it'll disappear entirely??).
For both of these I have no backpack upgrades, if that makes a difference in some way.
v1.6.15 build 24356. Windows 11 computer. The error log folder is empty and no error text in game. Using keyboard/mouse. Single player. English version. Unmodded
