If I haven't played since ~1.1, should I start over or pick up where I left off?

I played the game a lot soon after launch but never finished it. I've been meaning to pick the game back up for many years, but at this point should I just start over whenever I play again? Is there any content that would be missing/weird with that big of a leap? I think I only got partway into the second year, or maybe third? I never saw the ghost event that apparently happens at some point and never completed the community center stuff.

There's new farm layouts and a few new settings that you can change when starting the game, but if you don't care about trying a new layout, there shouldn't be anything you're missing. Really just a question of whether you want to experience the stuff you already saw again (I imagine you've probably forgotten a fair bit of what you did) versus jumping straight into new stuff.

It depends on how far you got into the game in 1.1, and how attached you are to that farm.

The new content is seamed in nicely to the game now. Villagers show up at your door to tell you about unlocked features (one of my saves saw 3 villagers in 1 day because of the massive update), and then you just keep farming or...whatever it is that you do.

Thanks for the info so far!

How much IS different? Was it mostly content added onto the end, or were aspects that affect the game overall added? Were there any mechanics/content in the early game added? Any events or anything that would be possible to miss?

I mean, there's a lot. You're kinda just going to have to play and find out for yourself. :)
Heart events got added all over the place as well as dialogue, end-game content. New booths for Pierre, and a whole host of other things.
Or you could look at the changelogs, but do beware that it's not spoiler-free.

Honestly, if it were me, I'd play your old save first, at least for a little bit. See what's new in a save where you're already established and not struggling to get started. Then, once you think you're up to speed on the things that have changed which you have access to, you will have a better idea yourself if you want to keep playing that one for a while or start a new one.

If you haven't played since 1.1, there will be new content at all stages of the game, but you're not going to miss anything by opening your old save, you may just get a few cutscenes all at once that you'd get spread over the first year on a new save.

Thanks all! Based on your info I'm going to stick with my existing save for the moment and then decide later if I want to play again from scratch, as imnvs suggested.
