It happened to me twice that during Qi's challenge of reaching level 120 in the mines with a higher difficulty, level 86 (a monster-ridden level, at least for me, always) sometimes does not give a ladder. If I'm correct, there was the sound of a bat screeching, which I assume might have been the last monster before a ladder appears, but the bat did not appear for ages, as if it got "lost". This happened twice, and sometimes it also doesn't happen, and a ladder appears. Can't recall if during those times there was a bat to slay.
