I'm not sure if this bug exists for others since I don't have any error logs to prove it and it only happened to me once (so far). At some point, while venturing down the mines (version 1.5 unmodified with the Shrine of Challenge activated), I reached floor 66 and completely annihilated the entire floor for any items but no staircase spawned on site. I already checked multiple times whether I left some stones or the stairs were just hidden from view. But nothing's definitely amiss except for the missing staircase..
