I've encountered an occasional bug on floors 40-80 of the mines' alternate version after activating the Shrine of Challenge or accepting the "Danger In The Deep" quest from Mr. Qi's Walnut Room: sometimes, forested mine floors do not spawn an exit ladder even after breaking all rocks (and slaying all monsters).
I believe this bug may be happening because some of the usual rocks appear to be transformed into fallen tree branches, and the fallen tree branches do not seem to ever spawn ladders the way rocks do ... so if one breaks all of the rocks before chopping all of the branches, the last rock (which I assume should always spawn a ladder) doesn't spawn a ladder since tree branches (which seem to be converted rocks) still exist on that floor.
One possible fix might be to ensure that fallen tree branches spawn ladders like rocks do; another possible fix might be to adjust the ladder-spawning chance check to account for rocks having been converted into fallen tree branches.

The bug can be worked around by exiting the mines and then re-entering them. However, this isn't always convenient, particularly if the bug occurs on a floor right before reaching an elevator.

- I did a forum search for similar bugs, and I found one thread which this report might be a duplicate of (https://forums.stardewvalley.net/threads/bug-mine-floor-with-no-exit.3946/#post-24255). There are not many details in that thread so I am unsure if it is the same issue or not — at least one post on that thread reports details inconsistent with this report — but I believe chances are high that it is the same issue. It might be in bad form, but I opted to create a new thread for this bug rather than contribute to that thread because I had many additional details plus a video to include, and that thread seems to have stalled out half a year ago.
