The wiki says this about the CalicoJack minigame:

I've seen this exactly once before. I don't know the exact odds of this happening but it happens and it is very rare. Has anybody else seen it?

I have never seen that before. That is so cool. Maybe it's cause I don't gamble enough. This definitely sounds like something that would be in the game.

I've seen it before. I'm not sure I gamble often enough for that to feel like a 'normal' rate of appearance, but my guess is it wasn't since I've seen it exactly once and never again. Not that I'm always at the casino, mind you. They don't even have a restaurant, nevermind a bar!
