ladders

Messages pop up when clearing an infested area, but I propose an option to always have an announcement. Would be helpful in multiplayer, nowhere else really... Perhaps an on/off switch in settings.

General question. When traversing in the Mines and Skull Cavern, we need to make ladders or holes appear in order to descend. To do this - unless there is already one uncovered - we need to break rocks, or kill enemies. I was wondering... Are the positions of ladders/holes predetermined each...
