When interacting with the prismatic shard chest in the Volcano Dungeon if you destroy the chest mid animation the chest will respawn when you leave the cave. Pressing A (for xbox) and A again will take the prismatic shard and also destroy the chest. Done on the xbox, v1.6.15 build 1 Single player.

2 March, 2025: I placed an Obelisk directly left of the mushroom cave on the standard farm and the top corner is not visible. I have tried moving it somewhere else and then putting it back but it did not fix it
